Facility Overview

Beachwood Pointe Care Center, a member of the Provider Services family of facilities, is a 140 bed skilled nursing facility located in Beachwood, Ohio at 23900 Chagrin Blvd. 

The team at Beachwood Pointe believes strongly in a resident centric environment, keeping the residents wants and needs at the core of every decision we make.  Our community specializes in caring for seniors with chronic illnesses and cognitive deficits.  Currently, we are embracing culture change by creating a person centered care environment.  In this environment our staff and residents work together promoting individuality, cultural expression and the celebration of life.   At Beachwood Pointe we feel that a resident’s choice should be at the heart of every decision that is made concerning their care or their environment.  Every day we see these choices at work through our nursing, dietary, activities and housekeeping departments. 

Clinical Excellence

Beachwood Pointe’s Clinical team is led by a geriatric nurse practitioner along with strong managers on every unit.  In order to provide top quality care a facility must make an investment in the front line staff.  Beachwood Pointe recruits only the finest Registered Nurses and qualified LPN’s, and maintains a scheduled staffing of S.T.N.A.’s no higher than a 1:10 staff to resident ratio.  All of our staff are well trained and are continually in-serviced on the issues that face seniors today.

Therapy and Rehabilitation

The clinical expertise of our rehabilitation department assists our residents in achieving their maximum potential, servicing both short-term and long-term care plans.  Our therapists carefully measure and document the resident’s progress and at the time of discharge ensure that all consumers receive the education necessary to continue their therapy at home and be successful.   Each resident’s wellness plan is developed cross functionally with an holistic approach. Our department relies on training, technology and their passion to deliver the best results. 

Physical Plant Renovations

Since July of 2006 Beachwood Pointe has undergone over $600,000 in renovations including new carpeting, wall coverings, whole house painting, new HVAC systems, resident room furniture and countless other equipment upgrades to nursing, dietary and therapy.  It is important to us that our physical plant upgrades mirror the ongoing clinical successes.

Culinary Services

At Beachwood Pointe, gone is the day of pre-prepared and institutionalized meals.  In response to the requests and concerns of our customers we’ve hired three chefs and transitioned to a scratch kitchen with a resident driven menu. Now all meals are served fresh to the residents at times of their choosing.  Emphasis is placed on knowing each resident’s likes and dislikes allowing us to produce quality meals served in family style that prompt socialization and create a sense of community for our customers, their families and our staff.  Our ability to adapt quickly to the needs and desires of our residents is what has elevated the care and compassion at Beachwood Pointe and has made us among the best in the area.

Resident Life & Recreation

Following our theme of person centered care, our resident directed calendars promote personal growth, community involvement and social opportunity. Programming encourages resident choice, creating an atmosphere for our residents to bring purpose to their lives and have some fun!  For our rehabilitation and long term residents current favorites include participation in Nintendo Game System “Wii”, opportunities to participate and lead cooking sessions, happy hour, dominoes, and many other purposeful events.

 A separate calendar is created for our specialized Alzheimer’s and Dementia floor, ”Garden Grove”, spotlighting maintenance of resident involvement through memory and sensory stimulation. Normalization tasks are integrated into daily activities and recreational event planning, keeping the participation level of each resident as a focus.

Staff & Resident Education

Beachwood Pointe’s staff members are constantly refreshing their skills and knowledge of today’s healthcare related issues with the assistance of our full time Staff Development Coordinator.  Our staff is in-serviced and tested to ensure that both the knowledge and the skill set are absorbed.

On-going unit and diagnosis specific training is mandated for all disciplines at Beachwood Pointe.  Upon initiation our CHF & COPD center all staff have been trained by our Nurse Practitioner to recognize and react appropriately to the clinical signs and symptoms. 

Families and residents are given the opportunity to expand their knowledge of health issues, government regulations, community awareness and facility functions. It is our belief that communication and educational support to families is essential to the overall wellbeing of our residents.
